Dear Friends:
The Greater Mercer Public Health Partnership (GMPHP), called by its mission and vision to create a healthier Mercer County, engaged in a Community Health Needs Assessment in 2012 to identify the health status of the population, with emphasis on those at higher risk.
Thanks to funding from Robert Wood Johnson Foundation New Jersey Health Initiatives, GMPHP, is led by United Way of Greater Mercer County, a coalition with Capital Health Hopewell, Princeton Healthcare, RWJ Hamilton, St. Lawrence Rehabilitation, local and county public health agencies working with regional organizations and community members to assess and address community needs. This collaborative effort brought together representatives of health and human service provider agencies along with leaders in all sectors of the community including government, education, housing, transportation, faith, and business to complete a collaborative health assessment to better understand vital health sustaining forces in the community. Its purpose was to prioritize those problems for which there are opportunities to create a positive outcome, and to develop and implement action plans to improve the health of everyone.
Our partners realize being recognized as leaders in providing the most comprehensive health care, public health and social services in the region is an honor, and it means that it is our responsibility to continually provide the highest degree of care for those we serve. The results of this study will assist us in our collaborative efforts to make a noticeable impact on the health of Mercer County. We are grateful to the support of all of our contributors who helped to make this community needs report possible.
